-Copy of ITIN applicant's passport
-ITIN Applicant's Certified copy of passport
  (issued by the Ministry of Foreign Affairs and Trade, US Embassy or   
   Ward Office)

-In the case of an ITIN renewal, the ITIN number already issued and
  the documents listed above
